The McHenry County Department of Health is investigating illnesses tied to the Crystal Lake Montessori School, asking anyone who ate food at a school event to complete a confidential survey regardless of whether they developed symptoms.

The illnesses under investigation occurred on or after Feb. 7 and are tied to the school, located at 3013 S. Country Club Road between Woodstock and Crystal Lake, or at any events sponsored by the school, according to a news release.

The investigation is ongoing and therefore information around the total number of cases of illness and the cause of illness was not available Saturday, the department said.

The survey is available at redcap.link/foodillnesssurvey and should take only a few minutes to complete.

The McHenry County health department also is asking that anyone who is associated with the school and became ill but did not attend any of the school-sponsored events on or after Feb. 7 also complete this survey.

Questions about the survey can be directed to a communicable disease nurse at 815-334-4500.
