James Kaczmarczyk, 20, of the 2400 block of South Sixth Avenue, Riverside, was cited at 6:01 a.m. Feb. 24 with consumption of alcohol by a minor in the 100 block of Fellows Court.
Deceptive practice
An incident involving a deceptive practice was reported at 9:49 a.m. Feb. 23 as having occurred at 3:14 p.m. Nov. 15, 2017, at Check Express Currency, 683 W. North Ave. A suspect reportedly cashed a counterfeit check.
Disorderly conduct
Ivan Chaidez, 19, of the 2800 block of Louis Street, Franklin Park, was cited at 2:41 a.m. Feb. 25 with befouling property and consumption of alcohol by a minor at CVS Pharmacy, 110 W. North Ave.
Battery
• Phillip V. Breckler, 30, of the zero to 100 block of Willow Tree Court, Elmhurst, was charged at 4:25 p.m. Feb. 27 with domestic battery in the zero to 100 block of Willow Tree Court.
• Erik Nowak, 23, of the 200 block of East Columbia Avenue, Elmhurst, was charged at 10:35 a.m. March 2 with domestic battery in the 200 block of East Columbia Avenue.
Drug paraphernalia possession
Daniel Todd, 21, of the 8500 block of Cedar Street, Fox Lake, was charged at 11:45 p.m. Feb. 25 with possession of drug paraphernalia, speeding and no valid driver's license at Route 83 and Canadian National Railway.
DUI
• John Winn, 48, of the 4300 block of Maypole Avenue, Chicago, was charged at 3:36 a.m. Feb. 24 with driving under the influence of alcohol and driving with license suspended at Park and Boyd avenues.
• Richard Peterson, 48, of the 17W200 block of Oakdale Drive, Bensenville, was charged at 8:28 p.m. Feb. 24 with DUI of alcohol, illegal parking in the roadway, improper lane use, unlawful possession of marijuana and possession of drug paraphernalia at Willow Street and North End Avenue.
• Elizabeth Gniatczyk, 53, of the 200 block of North Maison Court, Elmhurst, was charged at 7:34 p.m. Feb. 26 with DUI of alcohol, speeding, failure to yield to an emergency vehicle, disobeyed a stop sign, improper lane use, no insurance and resisting at Berteau Avenue and Third Street.
• Enrique Prado, 24, of the 300 block of North Hemlock Avenue, Wood Dale, was charged at 2 a.m. Feb. 28 with aggravated DUI of alcohol, aggravated driving while license suspended for DUI of alcohol and leaving the roadway at Fremont Avenue and York Street.
• Rene Lopez, 43, of the 4600 block of West 83rd Place, Chicago, was charged at 1:43 a.m. March 1 with DUI of alcohol in the 700 block of North York Street.
Forgery
• An incident involving forgery was reported at 9:59 a.m. Feb. 24 at McDonald's, 920 N. York St. A suspect reportedly attempted to pay for his purchase using a counterfeit $20 bill and stated he was not aware the bill was counterfeit and didn't know where he received it.
• An incident involving forgery was reported at 1:57 p.m. Feb. 27 as having occurred between noon April 26, 2016, and noon Aug. 29, 2017, in the 100 block of Poplar Avenue. The victim reportedly signed up with a company named "Ipayment," where the suspect receives a percentage of each transaction. The victim discovered multiple fees and learned two separate applications had been submitted after her initial signup. Her name fraudulently was signed and electronically filed, causing the new charges. The accounts have since been closed.
Identity theft
• An incident involving identity theft was reported at noon Feb. 24 in the 200 block of North Evergreen Avenue.
• An incident involving identity theft was reported at 12:52 p.m. Feb. 24 as having occurred at noon Feb. 15 in the 200 block of North Evergreen Avenue.
• An incident involving identity theft was reported at 5:25 p.m. Feb. 26 as having occurred at noon Dec. 19, 2017, in the 400 block of Montrose Avenue.
• An incident involving identity theft was reported at 1:56 p.m. Feb. 27 as having occurred between noon Jan. 7 and noon Feb. 27 in the 600 block of Armitage Avenue.
• An incident involving identity theft was reported at 2:07 p.m. Feb. 28 as having occurred at 8 a.m. Feb. 28 in the 100 block of Adams Street.
• An incident involving identity theft was reported at 11:52 a.m. March 2 as having occurred at 6:40 a.m. Feb. 26 at Elmhurst College, 190 S. Prospect Ave. The victim reported someone attempted to change the deposit information with the victim's employer on the victim's payroll account to have the direct deposit diverted to a different account.
• An incident involving identity theft was reported at 3 p.m. March 2 as having occurred between noon Feb. 12 and noon Feb. 26 in the 100 block of South Poplar Avenue.
• An incident involving identity theft reportedly occurred at 5:11 p.m. March 2 in the 500 block of Belden Avenue.

• A juvenile was cited at 12:50 p.m. Feb. 23 with unlawful possession of marijuana at York Community High School, 355 W. St. Charles Road.
• Justin Schoop, 21, of the 10300 block of Canterbury Street, Westchester, was cited at 11:47 p.m. Feb. 23 with unlawful possession of marijuana, possession of drug paraphernalia and loud muffler at St. Charles Road and Fair Avenue.
• Ryan Domeracki-Johns, 21, of the 1800 block of Lake Shore Drive, Romeoville, was cited at 4:12 a.m. Feb. 24 with unlawful possession of marijuana and possession of drug paraphernalia at York and Hale streets.
• Isalis Arroyo, 20, of the 300 block of West Kenilworth Avenue, Villa Park, was cited at 6:01 a.m. Feb. 24 with unlawful possession of marijuana, speeding, possession of drug paraphernalia and driving while license suspended at St. Charles Road and Route 83.
• Bryan A. Andrade, 20, of the 300 block of South Berkley Avenue, Elmhurst, was cited at 4:37 p.m. Feb. 26 with unlawful possession of marijuana in the 300 block of South Berkley Avenue.
• Joshua A. Linder, 23, of the 300 block of South West Avenue, Elmhurst, was cited at 2:23 a.m. Feb. 28 with unlawful possession of marijuana and possession of drug paraphernalia in the 300 block of South West Avenue.
• Aaron D. Lewis, 22, of the 1500 block of North Lotus Avenue, Chicago, was charged at 5:35 p.m. March 1 with unlawful possession of marijuana, driving while license suspended, expired registration and tinted windows at Grand Avenue and Lake Street.
• Darius Harris, 21, of the 5900 block of West Maple Avenue, Berkeley, was cited at 9:49 a.m. March 2 with unlawful possession of marijuana, speeding and operating an uninsured motor vehicle at Butterfield Road and Saylor Avenue.
• Hermez J. Abraham, 20 of the 8600 block of Kedvale Avenue, Skokie, was cited at 10:50 p.m. March 2 with unlawful possession of marijuana and disobeying a stop sign at First and Maple avenues. Emily Cooper, 20, of the 14800 block of Caletta Terrace, Oak Forest, was cited at the same time and date with unlawful possession of marijuana and possession of drug paraphernalia at the same location.
• Tyshun T. Byrd, 20, of the zero to 100 block of Prairie View Court, North Chicago, was cited at 11:38 p.m. March 2 with unlawful possession of marijuana and possession of drug paraphernalia at North Avenue and Indiana Street.
Property damage
• Two exterior windows and one exterior glass door reportedly were damaged between 3:40 p.m. Feb. 27 and 7:50 a.m. Feb. 28 at Churchville Middle School, 155 Victory Pkwy. The damage appeared to have been caused by a BB-type projectile.
• Contents of a package reportedly were broken at 9:35 a.m. Feb. 28 at Shipaholic Plus, 643 N. York St.

An incident involving suspicious circumstances was reported at 8:38 p.m. Feb. 23 in the 700 block of North Kenilworth Avenue.
Theft
• An incident of theft by deception was reported at 10:17 a.m. Feb. 26 as having occurred at 5 p.m. Feb. 23 in the 600 block of South Saylor Avenue. The victim reportedly wired money to a delivery service for the online purchase of two Siamese kittens but did not receive the kittens.
• A purse reportedly was stolen between 1:50 and 2 p.m. Feb. 27 at Elmhurst Hospital, 155 Brush Hill Road.
• An incident of theft by deception was reported at 9:05 p.m. March 1 in the 600 block of West North Avenue. The victim stated she paid for a phone from an individual, and when she received the package the phone was supposed to be in, the box was empty.
• Keys to a Chrysler Sebring were reported as having been lost or stolen at 6:29 p.m. March 2 at Elmhurst College, 190 S. Prospect Ave.
Tobacco possession
A juvenile was charged at 3:01 p.m. March 2 with possession of tobacco at a vacant gas station at 105 S. Cottage Hill Ave.
Traffic violation
• Michael A. Heatherly II, 31, of the 400 block of North Prater Avenue, Northlake, was charged at 7:21 p.m. Feb. 18 with driving while license suspended at North Avenue and Route 83.
• Norma Perez-Gomez, 43, of the 4800 block of West Nelson Street, Chicago, was charged at 7:27 a.m. Feb. 23 with leaving the scene of an accident, operating an uninsured motor vehicle and failure to reduce speed to avoid an accident at North and Larch avenues.
• Elisa Reyes, 30, of the 5000 block of South Kedvale Avenue, Chicago, was charged at 3:45 p.m. Feb. 23 with driving while license suspended and disobeying a traffic control sign at Robert T. Palmer Drive and Marion Street.
• Antonio Gilmore, 25, of the 1700 block of North Central Avenue, Chicago, was charged at 11:55 p.m. Feb. 23 with driving without headlights, no insurance and no valid driver's license at North Avenue and Indiana Street.
