Register for the vaccine

Online registration now is available for essential workers and individuals age 65 and older. Registering for a vaccine does not confirm a vaccination appointment time. It guarantees that someone from the health department will contact you in the future to schedule an appointment. Online forms can be filled out at www.jotform.com/tmccook/Bureau. The wait time for the vaccine could be lengthy, as the health department continues to be limited on its allotment of vaccines per week.
